Equilibrio is a single-player casual puzzle game in which players need to guide a ball to the exit of each level by tilting the playfield. To move the ball, players tilt the game environment using various controls depending on the platform, such as tilting the Wii Remote or leaning on the Wii Balance Board in the Wii version. The game features fragile balls that must be carefully navigated to avoid falling or touching deadly obstacles like spikes or acid pools. Set across 64 levels (or 60 on the iPhone), the game challenges players with a variety of objectives and increasing difficulty, which adapts to the player's skill level. It includes five different game modes, from a conquest mode where players unlock levels sequentially to more challenging Challenge levels. Players can skip levels if desired, and each level tracks its own high scores.
